You may schedule your appointment by phone or online via our online scheduler.

Cancellation policy: 24-hour notice to cancel or change an appointment is greatly appreciated.  A $25 fee may apply if less notice of cancellation is given.

*Same-day appointments are rarely available.  

*On-line booking for our Poplar Avenue office is available 24 or more hours before a desired appointment time.

*To ensure you have an appointment time convenient for you it is best to schedule in advance.  Consider a recurring appointment.
